Qualifications

Graduate from an approved LPN nursing school.

LPN DUTIES under supervision of RN Supervisor include:

1. Report to appropriate RN Supervisor upon beginning and end of shift.
2. Perform all LPN duties, skills and activities mandated by the institution's Policies and Procedures under supervision of assigned supervising Registered Nurse.
3. Report patient status to assigned supervisor.
4. Record clinical notes according to the institution's policies.

JOB RESPONSIBILITIES IN THE HOME

SCOPE OF PRACTICE: LPN DUTIES FOR SKILLED CARE include but are not limited to the following:

1. Assure the clients safety.
2. Provide reality orientation.
3. Establish a therapeutic relationship with the older person and the family and/or significant other.
4. Use mutual decision making with the older adult and the family.
5. Give general individual nursing care.
6. Provide for physical and emotional comfort.
7. Provide teaching for the older person and family.
8. Assist older person with active and passive exercises.
9. Administer medications per physician order (if trained and certified).
10. Observe, evaluate and report significant changes in the older person's condition to family, doctor and office.
11. Document all relevant findings.
12. Plan, prepare and serve therapeutic diets to the client.
13. Maintain clean environment for the client; clean up food and cooking areas.
14. Maintain and safeguard equipment and supplies.
15. Integrate care with other health services.
16. Escort client to scheduled appointments.
